Oral L. plantarum 299, in contrast, has failed to improve TNBS-induced colitis Kennedy et al. 2000 ; . Studies on the effects of prebiotics on experimental colitis are very scarce. Suggesting potential adverse clinical liver effects. In contrast, other compounds demonstrate only minor or no signs of hepatotoxicity in the animal species tested, yet cause an increase in serum liver enzymes in more than 10% of humans during early clinical trials reviewed in Fattinger et al., 2001; Pessayre et al., 1985 ; . Our study focused primarily on compounds that were not predicted to result in clinical hepatotoxicity, due to the unresponsiveness of the preclinical species. The common characteristic of the drugs used in this study is the preferential elimination from the body via the biliary pathway, which accounts for at least 50% of total drug clearance. We and others have hypothesized that hepatotoxicity in humans taking these drugs may be associated with drug-mediated inhibition of active canalicular transport of bile components, including, but not limited to, bile acids Kostrubsky et al., 2001; Stieger et al., 2000 ; . These drugs are likely substrates for active liver transporter-mediated uptake and efflux into the bile canaliculi. The transporters that participate in biliary drug elimination also transport endogenous bile components. Therefore, there is a potential for mutual inhibition of drug and bile acid efflux from the liver, resulting in an increase in drug and bile acids retained in the liver over time. We hypothesized that compounds showing greater inhibitory potency for bile acid transport will have a greater risk of being hepatotoxic. To address this issue, we have used an in vitro model of cultured human hepatocytes with an extensive canalicular network. We developed a competition assay between the drug and radioactively labeled bile acid to test whether canalicular efflux of taurocholate can be inhibited in a concentration-dependent manner, and whether this inhibition would correlate with clinical hepatotoxicity. For the current study, we have used six macrolide antibiotics with both high and low incidences of clinical hepatotoxicity. Our data indicate that drugs with greater hepatotoxic risk are stronger inhibitors of taurocholate transport in cultured human hepatocytes. Previously, Fattinger et al. 2001 ; observed an increase in rat serum bile acids after treatment with drugs that cause hepatotoxicity in clinic. These authors found a dose-dependent in.
16 have been most pronounced in the lung where the challenge was concentrated. The early beneficial effects of HRL-3 both on survival and lung injury are consistent with this. In contrast to intravenous E. coli however, intrabronchial challenge ultimately resulted in an established nidus of infection where activated neutrophils would have had an important protective role in the innate immune response. Inhibition of pulmonary neutrophil recruitment by HRL-3 with IB E. coli as was manifested by early reductions in lung lavage and histology neutrophil counts may have resulted in more extensive infection and secondary injury. Later reductions in survival rates and worsened lung injury with HRL3 at 168 h are consistent with this. Although neither blood nor lung bacteria counts were increased in animals receiving HRL-3, this may be because administration of antibiotics 2 h before laboratory measures in the present study altered the results of microbiology testing. It is also possible that the microbiology techniques employed were insensitive to the effects HRL-3 may have had on bacterial clearance. Consistent with a potentially adverse effect of HRL-3 on host defense in the present study however, in a recently published phase 2 clinical trial in trauma patients, another L-selectin directed monoclonal antibody was found to be associated with dose related increases in the rate of infection 40 ; . Alternatively however, the detrimental effects of HRL-3 on survival rates or lung injury during intrabronchial infection in the present study may have been because it either interfered with a later adaptive immune response necessary for the clearance of an intrabronchial nidus of infection or with subsequent reparative processes. The present findings with HRL-3 are consistent with prior studies performed in this laboratory in a large animal model, in which monoclonal antibodies directed at leukocyte integrin adhesion molecules improved survival with intravenous tumor.

At first glance, this retrospective study appears to confirm the immediate and longterm effectiveness of TrP injections in abdominal trigger points. The authors did not report how they defined TrPs or TrP injections. What criteria did they use to palpate abdominal TrPs? Did they determine the presence of a taut band and the most tender region within that taut band as suggested by Simons, Travell and Simons 7 ; ? The authors do not mention whether they attempted to elicit local twitch responses or whether they infiltrated the general vicinity of a point of tenderness. Intramuscular TrP injections are only effective if the needle elicits local twitch responses 24 ; . The number of follow-up injections and the time line of follow-up appointments varied greatly. With such a wide range, retrospective chart reviews do not provide sufficient assurances that reported long-term relief is primarily due to any particular intervention. The authors expressed surprise about the lack of effectiveness of adding a steroid to local anesthetics. This may indicate a lack of familiarity with the integrated trigger point hypothesis 7 ; . Steroids are most effective when treating inflammatory conditions and there is no current evidence that TrPs are associated with local inflammation. The use of steroids is not recommended for the injection of TrPs and may actually lead to destruction of muscle fibers. There is also no evidence that using long-acting anesthetics are more effective than short acting anesthetics in the treatment of TrPs. Long-acting anesthetics may actually be more myotoxic. Polyethylene glycol and lactulose are prescription laxatives that belong to the category of osmotic laxatives, which are poorly absorbed or nonabsorbed substances that increase the secretion of water into the intestine.
