There were no transactions with directors during the year ended 31 december 2006 other than as outlined in the "transactions with directors and executive officers, " section of the report of the leadership development and compensation committee and in note 32 to the consolidated financial statements.
Te ntat i ve r cove r y g closed 2004, concerns over sustainability of the economic recovery faded away. Tax cuts and fiscal policy appeared to have created a stability of growth at the historical average rate of three to four percent as the year ended. Consumers began to spend more as employment and incomes improved. Despite the oil hikes, inflation remained tame, and the Fed's modest and steady rate increases seemed just about right for the emerging economic recovery. These factors helped fuel the markets to eventually pick up where we left off in 2003, appearing to finally shake off the malaise that started the decade. two in a row The S&P 500 Index closed the year up 10.88% -- that's two back-toback years of double digit gains. Much of this gain, 7.19%, was toted up in the second half of 2004, and that period started with a third-quarter loss. This was not a speculative market, as valueoriented stocks proved to be the biggest gainers in the last six months of the year, but it was driven by swings in perceptions of energy prices and the election. International markets did well overall, but with outperformance concentrated away from the major exchanges of London, Frankfurt and Tokyo. Remarkably, the Treasury market rallied in the second half of the year, despite the Fed's four rate increases. lo o k our view 2005 has the makings of a decent year economically, with growth slowing to something around 3%. Consumers may spend less and save more in the face of higher interest rates, which we expect to result from continued Federal Reserve actions. We believe businesses will watch their cost structures closely, keeping inventories and job growth low. We will not be surprised to see periodic spells of concern over terrorism and Iraq drive financial markets in an uncomfortable imitation of a roller coaster, with jolts higher and lower along the way. As the recovery becomes more extended -- and investors turn more cautious -- market leadership may change, with stability of growth and earnings regaining a premium in investors' eyes.

According to the Mental Health Code, legal guardians and other substitute decision makers are to be included in all facets of care once services begin. Information about a recipient's rights must be shared orally and in writing with the recipient as soon as his condition permits and with any guardian immediately. A recipient aged 12 or older and any guardian must also be informed upon commencement of services of the right to designate a person or agency to receive notice should the recipient's rights ever be restricted. The recipient is allowed to select a preference for forced emergency treatment and the facility is to communicate a selection to any guardian 405 ILCS 5 2-200 ; . If any guaranteed right under the Mental Health Code is restricted, including the right to refuse medications, then the facility must promptly give notice to the recipient, his guardian, and to any person or agency so designated 405 ILCS 5 2-201 ; . Additionally, the recipient's guardian, if the recipient is under guardianship, must be informed of his or her right to refuse medications, and if such services are refused, they should not be given unless they are necessary to prevent the recipient from causing serious and imminent physical harm to themselves or others 405 ILCS 5 2-107 a ; . Service planning and decisions on proposed treatment with psychotropic medications and electroconvulsive therapy include the recipient's and the guardian's views: A recipient of services shall be provided with adequate and humane care and services in the least restrictive environment, pursuant to an individual services plan. The plan shall be formulated and periodically reviewed with the participation of the recipient to the extent feasible and the recipient's guardian.or any other individual designated in writing by the recipient. 405 ILCS 5 2-102 a ; . If the services include the administration of authorized involuntary treatment, the physician.or designee shall advise the recipient, in writing, of the side effects, risks, and benefits of the treatment, as well as the alternatives to the proposed treatment. The physician shall determine and state in writing whether the recipient has the capacity to make a reasoned decision about the treatment. The physician or signee shall provide to the recipient's substitute decision maker, if any, the same written information that is required to be presented to the recipient in writing. If the recipient lacks the capacity to make a reasoned decision about the treatment, the treatment may be administered.

Medication Choices In patients primarily concerned with depression, addressing pain may help manage treatment-resistant depression. Dr. Glick supported the use of agents that boost both serotonin and norepinephrine such as venlafaxine and duloxetine.49 Higher doses of duloxetine 120 mg day ; may produce a better analgesic effect than lower doses.39, 50, 51 In Dr. Glick's clinical experience, at least 150 mg day of venlafaxine are needed to achieve an analgesic effect. Dr. Glick recommended the TCA nortriptyline for pain and depression, dosed in an antidepressant range of 50 to mg day, but also cautioned that the dose should be reduced in older patients because of the risks of sedation, falls, and heart arrhythmia.52 Duloxetine carries a specific indication for use in neuropathic pain conditions such as diabetic neuropathy, but can be helpful for patients with fibromyalgia and other chronic pain states.39, 50, 51 Dr. Glick stated that specifically for fibromyalgia or neuropathic pain, a higher dose 120 mg day or 60 mg p.o. b.i.d. ; may provide a greater benefit than the reduced dose of 60 mg day most commonly used for depression. Similarly, the anticonvulsant pregabalin produces a better analgesic effect at a dose of 600 mg day t.i.d.53 Pregabalin does not have a specific antidepressant effect but can be used in patients primarily presenting with pain, 54 either alone or in concert with an antidepressant. As with the primary management of depression, when treating depression with comorbid pain the choice of medication can be influenced by the.
Arterial stiffness and central aortic pressure were measured noninvasively by the technique of pulse wave analysis using the SphygmoCor apparatus Version 6.01; AtCor, Sydney, Australia ; as developed by O'Rourke and Gallagher 22 ; . All measurements were taken from the radial artery at the wrist using a micromanometer SPC-301; Millar Instruments, Houston, TX ; , applying the principle of applanation tonometry to flatten the artery by gentle pressure. Data were collected directly into a desktop computer and processed by the system software to allow accurate on-line recording of the radial artery waveform. The corresponding aortic pressure waveform can then be generated from an averaged radial artery waveform derived from 20 sequentially recorded radial artery waveforms ; using a validated transfer factor 2224 ; . Computerized analysis of the central waveform allows determination of the augmentation, the augmentation index, central aortic pressure, and the timing of wave reflection. The augmentation index is defined as the difference between the first and second peaks of the central arterial waveform, expressed as a percentage of the pulse pressure 22 ; . The timing of the reflected wave, determined by calculating the time between and dulcolax.

There have been marked changes to the experiences of living with HIV since the widespread implementation of highly active anti-retrovirals, ARVs ; in Australia. These generally more positive experiences may result in better employment prospects and consequently better social and economic circumstances. We examine the current employment experiences of People Living with HIV AIDS PLWHA ; who completed Futures 5 F5 ; in 2005 and were aged 65 years or less. Their health status, wellbeing and experiences of discrimination in the workplace are examined. Of the total of 936 PLWHA in Futures 5 who were 65 years or less at the time of survey, 33.7% were employed full-time, 17.1% reported part time employment, and 3.8% were students. Of those unemployed 10.9% ; or not working 19.5% ; at the time of the survey, 19.1% reported having stopped working at the time they were diagnosed with HIV. Adjusting for age, employed PLWHA had significantly better self reported health and better self reported well being; they also reported significantly higher CD4 counts. Employed PLWHA were less likely to be currently on ARVs and twice as likely to report never having been on ARVs. We compared F5 findings with the findings from the previous four studies conducted in 1997, 1999, 2001 and 2003 to gain a comprehensive picture of how employment status and experiences have changed over time for PLWHA in Australia. Full-time employment rates remained constant between 1997 and 2003 but were significantly higher in F5. Part time rates have remained relatively stable over the surveys and duragesic. Some are written by those with advanced degrees--for example, Frederick Goodwin and Kay Redfield Jamison's Manic-Depressive Illness New York: Oxford University Press, 1990; a second edition is forthcoming in 2007 Peter Whybrow's A Mood Apart: Depression, Mania, and Other Afflictions of the Self New York: Basic Books, 1997 and E. Fuller Torrey and M. B. Knable's Surviving Manic Depression New York: Basic Books, 2002 ; . Others are autobiographies by adults with the disorder. For example, Electroboy: A Memoir of Mania by Andy Behrman New York: Random House, 2002 ; , is an edgy, engaging memoir of his life in New York City and of his experiences dealing with the complications of bipolar disorder, including treatment with electroconvulsive therapy. Kay Redfield Jamison's Unquiet Mind: A Memoir of Moods and Madness New York: Alfred A. Knopf, 1995 ; and Patty Duke's Call Me Anna New York: Bantam, 1987 ; are also highly recommended reading. Identified, remains uncertain; but evidence exists that all play a part in memory, appetite, the control of movement, and, especially, the modulation of pain.1 With respect to the last of these, the sites of cannabinoid action in the central nervous system are confined to specific areas, most of which--the dorsal horn of the spinal cord, for example2 3--are involved in processing pain signals. Clear parallels exist between cannabinoid and opiod receptors, and evidence is accumulating that the two systems sometimes interact, 4 and may operate synergistically. One unproved but intriguing idea is that endocannabinoids may set the "analgesic tone" of the body, with the level of their production acting as a kind of pain thermostat. The perception of pain is controlled by neurotransmitter systems within the central nervous system, but peripheral tissues also have mechanisms for relieving and preventing pain. Cannabinoids may therefore have two distinct roles in relation to pain. The evidence comes from animal experiments showing that cannabinoids lower the response of pain neurones in the spinal cord and also in parts of the thalamus in the brain. The possibility that cannabinoid receptor subtypes act synergistically hints at a potentially valuable strategy: the development of a new class of analgesic drug comprising a mixture of synthetic CB1 and CB2 agonists. Alternatively, devising agents to slow the breakdown of natural cannabinoids might potentiate their analgesic effects.5 The brain has many more CB1 than opioid receptors, and interest in the therapeutic potential of cannabinoids has prompted the development of several synthetic variants, of which dronabinol is one.1 Many of these compounds bind to both CB1 and CB2 receptors, but differences between the two suggest that it should be possible to design drugs that would attach to only one of them. This might offer a means of producing more selective biological actions, including analgesia without some of the psychotropic effects that might disturb people whose wish was to be pain-free but not "high." A review by the US Institute of Medicine has commented on how little we know about cannabinoids in comparison with opiates.2 A comparison between the history of research into the two groups, the review added, "suggests good reason for optimism about the future of cannabinoid drug development. Survival with conventional chemotherapy is low. The 5-year OS ranges between 0% and 10% in small series reported.2, 3 In an attempt to improve the outcome for patients with primary progressive HD, HDCT followed by ASCT has been increasingly used.4-7 We retrospectively analyzed Hodgkin lymphoma patients with PD registered in the GHSG database to determine prognostic factors and outcome after salvage treatment. Another aim of this study was to identify patients who are candidates for more aggressive treatment modalities. The collection of data was based on the definition of primary progressive disease as disease progression during induction treatment or within 90 days after the end of treatment.2, 6, 7, 14 Our results show that the long-term outcome of patients with primary progressive HD is disappointing. The 5-year FF2F and OS rates for all patients were 17% and 26%, respectively. This predominantly poor prognosis emphasizes the need for more effective first-line treatments that reduce primary progression rates.10 The 5-year OS of patients progressing after presenting in intermediate or advanced stages at first diagnosis did not differ significantly, indicating that one of the most important determinants of outcome in primary treatment is less important when disease progression occurs. An important predictive factor correlated with OS was the Karnofsky performance score at the time of disease progression. The 5-year probability of OS with Karnofsky performance scores of 90% or higher and less than 90% were 34% and 6%, respectively. The age of the patient was also an important factor for OS. Unfortunately, older age and poor performance status may discourage the use of more aggressive treatment modalities in these patients. Moreover, attainment of a temporary remission with first-line chemotherapy was a significant predictor for OS. Patients never achieving a temporary remission during first-line chemotherapy had an 18% lower survival probability than the remainder, indicating the biologic aggressiveness of the disease in this subgroup. However, multiple randomized trials in the 1980s and 1990s have changed the role of chemotherapy in lung cancer, such that by the middle of the next decade, it may be that only patients with stage IA nonsmall-cell lung cancer are not considered as candidates for chemotherapy.
