There have been marked changes to the experiences of living with HIV since the widespread implementation of highly active anti-retrovirals, ARVs ; in Australia. These generally more positive experiences may result in better employment prospects and consequently better social and economic circumstances. We examine the current employment experiences of People Living with HIV AIDS PLWHA ; who completed Futures 5 F5 ; in 2005 and were aged 65 years or less. Their health status, wellbeing and experiences of discrimination in the workplace are examined. Of the total of 936 PLWHA in Futures 5 who were 65 years or less at the time of survey, 33.7% were employed full-time, 17.1% reported part time employment, and 3.8% were students. Of those unemployed 10.9% ; or not working 19.5% ; at the time of the survey, 19.1% reported having stopped working at the time they were diagnosed with HIV. Adjusting for age, employed PLWHA had significantly better self reported health and better self reported well being; they also reported significantly higher CD4 counts. Employed PLWHA were less likely to be currently on ARVs and twice as likely to report never having been on ARVs. We compared F5 findings with the findings from the previous four studies conducted in 1997, 1999, 2001 and 2003 to gain a comprehensive picture of how employment status and experiences have changed over time for PLWHA in Australia. Full-time employment rates remained constant between 1997 and 2003 but were significantly higher in F5. Part time rates have remained relatively stable over the surveys and duragesic.

One unproved but intriguing idea is that endocannabinoids may set the "analgesic tone" of the body, with the level of their production acting as a kind of pain thermostat. The perception of pain is controlled by neurotransmitter systems within the central nervous system, but peripheral tissues also have mechanisms for relieving and preventing pain. Cannabinoids may therefore have two distinct roles in relation to pain. The evidence comes from animal experiments showing that cannabinoids lower the response of pain neurones in the spinal cord and also in parts of the thalamus in the brain. The possibility that cannabinoid receptor subtypes act synergistically hints at a potentially valuable strategy: the development of a new class of analgesic drug comprising a mixture of synthetic CB1 and CB2 agonists. Alternatively, devising agents to slow the breakdown of natural cannabinoids might potentiate their analgesic effects.5 The brain has many more CB1 than opioid receptors, and interest in the therapeutic potential of cannabinoids has prompted the development of several synthetic variants, of which dronabinol is one.1 Many of these compounds bind to both CB1 and CB2 receptors, but differences between the two suggest that it should be possible to design drugs that would attach to only one of them. This might offer a means of producing more selective biological actions, including analgesia without some of the psychotropic effects that might disturb people whose wish was to be pain-free but not "high." A review by the US Institute of Medicine has commented on how little we know about cannabinoids in comparison with opiates.2 A comparison between the history of research into the two groups, the review added, "suggests good reason for optimism about the future of cannabinoid drug development.

Survival with conventional chemotherapy is low. The 5-year OS ranges between 0% and 10% in small series reported.2, 3 In an attempt to improve the outcome for patients with primary progressive HD, HDCT followed by ASCT has been increasingly used.4-7 We retrospectively analyzed Hodgkin lymphoma patients with PD registered in the GHSG database to determine prognostic factors and outcome after salvage treatment. Another aim of this study was to identify patients who are candidates for more aggressive treatment modalities. The collection of data was based on the definition of primary progressive disease as disease progression during induction treatment or within 90 days after the end of treatment.2, 6, 7, 14 Our results show that the long-term outcome of patients with primary progressive HD is disappointing. The 5-year FF2F and OS rates for all patients were 17% and 26%, respectively. This predominantly poor prognosis emphasizes the need for more effective first-line treatments that reduce primary progression rates.10 The 5-year OS of patients progressing after presenting in intermediate or advanced stages at first diagnosis did not differ significantly, indicating that one of the most important determinants of outcome in primary treatment is less important when disease progression occurs. An important predictive factor correlated with OS was the Karnofsky performance score at the time of disease progression. The 5-year probability of OS with Karnofsky performance scores of 90% or higher and less than 90% were 34% and 6%, respectively. The age of the patient was also an important factor for OS. Unfortunately, older age and poor performance status may discourage the use of more aggressive treatment modalities in these patients. Moreover, attainment of a temporary remission with first-line chemotherapy was a significant predictor for OS. Patients never achieving a temporary remission during first-line chemotherapy had an 18% lower survival probability than the remainder, indicating the biologic aggressiveness of the disease in this subgroup.
